Name & story
The name 下髎 Xialiao means "Lower Foramen" or "Lower Bone-Hole". It is the fourth and lowest in a family of four sacral points on the Bladder Meridian (BL) — Shangliao BL-31, Ciliao BL-32, Zhongliao BL-33, and Xialiao BL-34 — each named for the sacral foramen it sits over: upper, second, middle, and lower. Together they form a group sometimes called the "Eight Liáo" (with their pairs on both sides). Xialiao is the deepest and most inferior of the four, reaching into the lowest corner of the sacrum, where the pelvis gathers its most rooted and private functions.

BL-34 belongs to the Bladder Meridian (BL). It is part of the celebrated "Eight Liáo" group — the four pairs of sacral foramen points (BL-31 through BL-34) — which together govern the lower pelvic region and are collectively considered some of the most important local points for gynecological, urological, and anorectal conditions.
Five-element dynamics
The Bladder Meridian (BL) belongs to the Water element, paired with the Kidneys. The sacral foramen points sit at the very base of the spine, over the sacrum — the bone that anchors the pelvis. In Chinese medicine, this region is closely linked to the lower Jiao (Lower Burner), the body's lower chamber, which houses the Kidneys, Bladder, uterus, and intestines. When Qi and Blood (Xue) stagnate in this lowest chamber — whether from Cold, Dampness, or simply from the demands of childbearing and aging — the Eight Liáo points, and BL-34 in particular, help move what has become stuck and restore the smooth flow of the lower Jiao.
Location
BL-34 is found over the fourth posterior sacral foramen — the lowest of the four sacral foramina on the back of the sacrum. It lies below BL-33 (Zhongliao), close to the tip of the sacrum. Locating the row of sacral foramina and counting down to the fourth (lowest) one leads you to BL-34.
Anatomy & fascia
The point is located over the fourth posterior sacral foramen, through which the needle can enter to reach the sacral canal region.
Needling
The needle is inserted perpendicularly, directed into the sacral foramen.
Safe depth
1–1.5 Cun.
Moxa, cupping & Tui Na
Moxa is well suited to this point, particularly when the underlying pattern involves Cold or Dampness in the lower Jiao — for example in cold, cramping menstrual pain or urinary difficulty with a Cold pattern. Warming the sacral region with a moxa stick or warm needle technique can help move stagnant Qi and Blood (Xue) and dispel Cold. Local massage over the sacrum can be used as a home adjunct.

Often used together with the other sacral foramen points — BL-31 (Shangliao), BL-32 (Ciliao), and BL-33 (Zhongliao) — as part of the Eight Liáo group for comprehensive treatment of lower pelvic conditions. BL-32 (Ciliao) is especially closely paired with BL-34 for gynecological and urological disorders.

The golden tip
For lower back and sacral discomfort, a warm compress or gentle massage over the lower sacrum — the triangular bone just above the coccyx at the base of the spine — can bring relief. Finding the area of BL-34 precisely at home is difficult, but warming the whole sacral region with a heat pack is a safe and soothing self-care option, especially when the discomfort feels cold or achy in nature.
